Error msg while adding new block

I can't add any new block. I got the below error message while trying to add any new block:

user error: Duplicate entry '' for key 3 query: INSERT INTO boxes (title, body, info, format) VALUES ('Whatever Title', 'Whatever Body', '', 3) in /var/vhosts/manalaa.net/hosting/includes/database.mysql.inc on line 66.

warning: Cannot modify header information - headers already sent by (output started at /var/vhosts/manalaa.net/hosting/includes/common.inc:384) in /var/vhosts/manalaa.net/hosting/includes/common.inc on line 192.

Please help

Problem Solved. It wasn't a problem indeed. Block Description shouldn't be NULL however nothing indicate this.

BTW, I replied on ur last comment as soon as u post it but i don't why it's not published.
